GP practices in England are set to receive an average £3,000 per year in incentive payments to prescribe weight loss drugs, following changes to the GP contract starting in April. The government has added weight loss drug prescribing to the GP incentive framework for the first time. Under the new contract, practices can earn bonuses for prescribing Mounjaro to the maximum number of patients who meet strict eligibility criteria.
This mirrors previous incentive schemes used to improve areas such as vaccination uptake, dementia care, and cardiovascular prevention. Ministers say the goal is to ensure patients who could benefit from medical weight loss support are not being missed.
NHS Access to Weight Loss Injections Is Still Strictly Limited
Despite Mounjaro becoming available on the NHS in 2025, access remains tightly controlled. Current prescribing is generally restricted to patients with severe obesity, typically those with a BMI over 40 alongside specific health conditions.
Eligibility is expected to widen next year to include patients with a BMI over 35, with lower thresholds for certain ethnic groups. Even then, projections suggest that only around 220,000 patients will be receiving Mounjaro through the NHS by 2028.
This is in stark contrast to current usage, with over one million people in the UK estimated to be using weight loss injections, most of whom are accessing treatment privately.
Why Wegovy Is Not Prescribed by GPs
Unlike Mounjaro, Wegovy is not prescribed directly by GPs. Instead, it is accessed through specialist NHS weight loss services, creating additional referral steps and longer waiting times.
This difference in prescribing pathways has contributed to inconsistent availability across England and ongoing delays for many patients seeking treatment.

Obesity health organisations have broadly welcomed the incentives as a positive signal but warned that expectations must be managed. NHS eligibility rules remain unchanged, and the bonuses do not widen access to those who fall outside clinical criteria.
Professional bodies have also raised concerns that expanding prescribing responsibilities could increase GP workload and create frustration among patients who are not eligible for treatment.
